I have one of our providers inquiring about billing telephone calls to patients in regards to genetic testing results. Would this be billed with an office POS 11? Or does it fall under the Telehealth POS 02 or 10?

All E/M services whether telephone or audio-video, must be medically necessary. Making a phone call to a patient about test results is not a billable service unless there was evaluation and management. Here is a link to an article by Terry Fletcher which addresses this - https://www.terryfletcher.net/2024/05/13/telehealth-fact-sheet-and-e-m-tips-for-2024-2/
Thank you for responding!!! For those calls (audio only) that truly have an E&M service involved, do know or have a link, if they also use the telehealth (audio/visual) POS 02 or 10?

Is it necessary to document the actual specific time spent on audio-only telehealth 99441-99443 codes? As the time for each code is already in the CPT definition just wondering if that is a requirement. Thanks in advance!

The audio only 99441-99443 are leveled based only on time, not MDM. If the records do not contain the time spent on the phone, what would you use to support the code?
PS - the times for audio 99441-99443 are the minutes on the phone call only, not all time spent on the encounter that day.

Do you utilize the POS 02 (or 10) for these codes just like the audio/visual telehealth codes?

Telehealth is like the wild west and every payor sets their own policy. TYPICALLY 02 or 10 depending on patient's location is what most payors want. The same "follow the payor's guideline" applies to if and what modifier is needed.

That is going to be payor-specific, you will have to check each payor's policies. Most commercial payors need POS 02 or 10 as the case may be, but not all. Medicare and Medicare Advantage Plans typically need POS 11 for these codes, but again, you will need to check with your specific Medicare Plan.

Thank You! Yes, that makes sense!
