I asked ACOG this question and this was their response
The diagnosis to be assigned will depend on exactly what took place during the encounter. If a contraception discussion was held concerning multiple options (of which sterilization was one option), the appropriate diagnosis is Z30.09. If the only topic during this encounter was sterilization, then Z30.2 is the appropriate diagnosis.
Z30.02 is for a discussion of natural family planning methods, and would not be appropriate for a sterilization consult.I have used Z30.02 for the discussion and Z30.2 for the actual procedure performed
