Our office keeps seeing CARC: IR: Federal on the bottom of remits, but cannot get an answer as to why. Any ideas? Thanks!

Is this for Medicare claims or are you seeing it on Commercial claims too?
On a semi-unrelated note: I tried to Google and stumbled on a good example of why AI still has a long way to go before it can replace humans entirely. According to the Google search AI summary, your claim denial was due to military camouflage. 
(Not really a fan of how Google now automatically shows these AI summaries at the top of search results. As you can see, accuracy can be... questionable.)

We have seen this on Commercial BCBS plans that are secondary to Medicare, and TN Medicaid Plans.
