In the past, providers have indicated that if a patient has a hyperplastic polyp it is not high risk so they bring patient back in 10 years. When they return for follow up colonoscopy in 10 years it is screening, not personal history of polyps. Now that we have the new code of Z86.0102, should it be coded first or should it be Z12.11 and then Z86.0102? and would you use G0105 or G0121 if no findings? I guess I don't understand why a specific code was created for Personal history of hyperplastic polyps if it isn't relevant. In an AskMueller webinar book it states that only Z12.11 is to be used with G0121. Thank you for your help.
 
depends on the insurance, I'd just stick with Z12.11 in first position. Avoid and kick backs, as for HIGH RISK. If you doc is saying NO to high risk, then it's not. But that dx does fall under HIGH risk.

If, hyperplastic polyps are noncancerous, why did they create the Z86.0102 code? Especially Z83.711?

If the patient has a polyp located in colon or rectum or where ever see dx block D12 dx series & location poly found, then use last dx Z12.11. If no polyp located just add dx K63.89 Disease of intestine, or if pt has another problem like K59 or R19 then add dx Z12.11.
