Provider performed fulguration of endometriosis of right ovary after completing patients c/s. OP note reads

"There a small left paratubal cyst of less than 1 cm, which was left alone. The right ovary has some superficial endometriosis hemosiderin deposits which were fulgurated completely."

Is this separately reportable or should I bill the c/s with modifier 22?

I would bill with a modifier -22 as there is no open procedure code for this procedure, only a laparoscopic one. Or of course you could bill an unlisted procedure. Code 49186 would not be an appropriate code to bill for open endometriosis removal in my opinion.
