Our doctors at one of our offices wants us to get their NP credentialed to see patients without them being present. My question, is once this is all set up, can we go back and forth from "incident to" billing when the MD is in the office to billing without supervision when they are not in the office? Or do we have to stick with one or the other? I hope this question makes sense. :-)

The NP being able to bill independently doesn't change whether a specific visit can be billed as Incident-To.

As long as ALL the Incident-To requirements are met. It's more than just the physician being physically present in the office, of course.

Here's a pretty thorough fact sheet on Incident-To billing. (It's from CGS Medicare, which is my MAC, but the fact sheet is based on the Medicare guidelines and broadly applicable even if CGS isn't your MAC)
