Question:
Regarding CPT II codes (quality codes). In our clinic, we report quality codes such as 1125F for “pain present” and 1126F for “no pain. When a patient’s smoking status is documented as “smoker” or “current smoker,” we capture that information. However, if smoking is documented only under the patient’s smoking history, do you all report it only when CPT II quality codes are required, or should it also be coded on routine visits that do not require CPT II codes?

If the patient is a smoker, instead of using a CPT II, we report it with a dx code. i.e.: Z72.0: Tobacco use (current use, no dependence or addiction noted) or a code from the F17 category.
