0256T has been deleted for 2013. The new code is 0257T, it is listed in the 2013 CPT book.

Dee I am looking for documenation on the code when it was first added to the CPT manual. The new code is actually 33361. The CPT Changes manual gives a better description of the code than the CPT manual. Thanks
