1% Lidocaine with propofol administration

For our endoscopic patients. A 1% lidocaine is mixed with propofol with administered via syringe. Is the lidocaine a separately chargeable item? Does the physician order
have to specificaly state the mixture? Does "standard of care" cover this without an order?
 
There is no code for lidocaine as part of an injection. The providers at my clinics often do joint injections with lidocaine and Kenalog, the lidocaine is not billable.

It is not a particularly expensive drug. If the cost is a concern you could attempt to bill as an unclassified drug, but prepare to appeal and send medical records to insurance.
