Wiki 11042 Help please

If a doctor debrides two separate wounds on the same foot would they be included in the same CPT 11042 since it says first 20 sq cm or less. Or would he "add" on the 11045 code for the second wound? Thanks
ps. What if they were two separate areas of the body?

You add up the total sq cm for all of the ulcers that were debrided of the same depth and use the appropriate code.
