11055/11056 denial for last date seen

I have recently received mass denials for 11055/11056 for last date seen - however when I look at the claim the LDS is on there but it is the last date seen from our office- am I over looking something or have the guidelines changed?
 
I've been getting them recently too. I've just been resubmtting them. However, if the pt is diabetic its better to use the DLS from the referring or endocrinologist/pcp
