Looking for guidance around using 1111F at the time of discharge. Can a provider bill out 1111F when they perform a medication reconciliation at the time of DC? My question is related to the HEDIS measure TRC Medication Reconciliation.

According to the measure guidance from NQF, 1111F is only appropriate for the ambulatory setting. It shouldn't appear on any inpatient claims, whether it's facility or profee. The only person who should be billing this is the provider who does the follow-up office visit.
 
Can a nurse do a medication reconciliation over the phone if the patient refuses to to be seen for hospital follow up visit? And, can a claim be filed with just the 111F code(cpt code ll) if the Provider reviews and updates the patient's medication list?
