Hello, I have a provider listing "debridement of fungal toenails performed x 10". Would this be sufficient for documentation of how many and which nails were debrided?
Unfortunately, no it is not. The physical exam needs to detail the nail findings, ie: long, thick, lyrics, dystrophic etc and which nails are that way. It should also detail which nails are only required trimming because they are not thick. The procedure should detail how the nails were treated such as which instruments etc.
