Anesthesia: The patient was injected with 5 mLs of 0.5% Marcaine plain.
Prep: The site was prepped with Betadine under sterile conditions.
Right Foot: 1st toe: , entire toenail was removed.
Dressing: a sterile compression dressing was applied.

11730(Avulsion of nail plate, partial or complete, simple; single)
11750(Excision of nail and nail matrix, partial or complete (eg, ingrown or deformed nail), for permanent removal)

Kindly clarify

The provider doesn't state that they nail matrix was removed, which is what makes it a permanent removal. You can either query the physician to clarify if the removal was a permanent removal or you can go with the 11730. As the documentation stands, 11730 is the correct code.
