When coding for 11730 should diagnosis code L03.032 be primary or should L60.0 be primary as most insurances will not cover it unless the ingrown is accompanied by an infection?

I have not found that to be the case. L60.0 should be enough for all payers. Many, though, are denying because the procedure was performed previously. If you get a denial, share the details, that way I can be of more help.
