My provider repaired a recurring hernia and did an adjacent skin transfer from the lip to the trunk the diagnosis code that he reported was 553.21 (incisional hernia w/o ob and gangrene) medicare denied this code for medical necessity and i can't seem to find a policy for this code, does anyone have a site that i can find out some more information on the medical necessity of this code? I actually tried to find a dx code for skin transfer but was unsuccessful any ideas would be appreciated.
