I am new to Pedi coding. 15 month old well visit performed and pneumococcal conjugate PCV13 vaccine given . During exam reducible hernia is notated(no treatment planned until pt is 4 yrs old is documented. Acute sinusitis is diagnosed and rx given. Would this be a well visit with an e/m? Or is the sinusitis minor and included in the payment for the well visit code?

acute sinusitis is diagnosed during the wellness visit ,it should be coded with e/m level as well as preventive visit code,because a new problem is identified and px has given apart from well child check and you need to append 25 modifier to e/m 25 as it is an extra service provided along with preventive visit.Hope this helps.
