We have a Convenient Care clinic that performs radiology services. Patient's may take their order for radiology services and have them performed. We bill for these services on a 1500. We are having an issue because Box 31 requires the name of the rendering physician. There is not a physician on site. We have tried to bill Medicare with our facility as the rendering provider in box 31 and they denied the claim. We have tried billing Medicare with the box blank, as other insurances have accepted and paid the 1500 with box 31 blank and they have denied those claims also. Any suggestions?
