If you have 2 areas on the same foot would you bill 15365 X2? If they do one on the right foot and one on the left foot bill 15365 X2 also. I think I am getting myself confused.

I think it depends on the size ...15365 is for the first 100 sq cm, then if any larger area of the same anatomical site is grafted then the add on code is 15366 for each addt'l 100 sq cm.
Might see if others have other opinions....
 
I would not bill X2. I agree that it is not based on the number of ulcer but the size of the combined ulcers.

Rumor has it that the procedure code for dermagraft is changing as of Jan. 1. Has anyone heard anything?

Peggy you are right only for Medicare the new code is G0440 for Apligraf/Dermagraft intial 25 sq cm and G0441 for each additional and the golbal periods have been dropped. These codes are for Physcians only Hospitals and facilities will still bill the 15365 and 15366 for Dermagraft, 15340 and 15341 for Apligraf
 
Thank you for the info on the G codes! I hadn't seen that info yet. Have you heard anything about a change in 15340 at all? One of our docs uses that code on occasion for grafts also.
