As a preface, I am only coding for the anesthesiologist, but I still need to use dx codes and have the CPT code crossover to the ASA code.

This pt went into labor at 17 wks and delivered the twins vaginally. They were born alive, but passed within 30 minutes. Of course, the medical records I have access to are a mix of "spontaneous abortion" to "NSVD" (quite annoying). I am still a new coder, but I'm wanting to code O60.12x1 and O60.12x2 with Z37.2. Would that be accurate? Or do I code as a spontaneous abortion because of it being 17 weeks?

Prior to 20 weeks 0 days, ACOG states to use E/M codes. CPT books states E/M codes 99201 - 99233 under Abortion category. It doesn't use miscarriage wording - just abortion. Patient experienced complete (spontaneous) abortion at 17 weeks. Diagnosis code could possibly come from O03.8x for any complication or O03.9 area (miscarriage NOS or spontaneous abortion NOS).
