I am trying to bill a state funded cmo for a well check and, on the same DOS, bill the 17250 for cauterization of the umbilical granuloma. Is this possible?
 
Add modifier 25 to preventive service. Also make sure granuloma is documented on exam and procedure is properly charted. Happy billing! Lburke cpedc
