I have two quick questions:
1.) For 99214 it is okey to take one element from one system and two elements for the other system and three elements in the other system as long as the total is 12 elements or it should be two elements from each system to make a total of 12?
2.)How about for 99204 for a total of 18 elements,it should be 2 elements from each system only not a breakdown as mentioned above?

The 1997 Guidelines state the following about a detailed exam:

Detailed Examination-should include at least six organ systems or body areas. For each system/area selected, performance and documentation of at least two elements identified by a bullet is expected. Alternatively, a detailed examination may include a performance and documentation of at least twelve elements identified by a bullet in two or more organ systems or body areas.

This is located on page 12 of the guidelines.
 
The comphrensive exam under the general multisystem 97 exam requires at least 2 bullets in 9 different systems. If you use the specialty exams they have more specific info on which bullets must be done.

Just to add to what Laura said. For comprehensive, the two bullets must be from 9 different systems. If they are not from nine different systems, then you can't count it as comprehensive.
