I need help deciding if this history is problem focused or expanded. "An established patient comes in to see me with a chief complaint of a rash on his chest. It started yesterday morning." In order for it to qualify for expanded history it needs at least one ROS. Can the mere mention of rash on the chest be considered a ROS or in this case is there no ROS at all? Is this a problem focused history or expanded history?
