Does anyone know the documentation guidelines for a first assist at surgery? I have a surgeon who is documenting the name of her NP as first assist but there is no further reference on the operative report.
 
Some explanation of how the NP assisted

There should be some reference throughout the operative report of the role of the assistant. That being said, our Compliance office has blessed the use of "we" throughout the report to signify that the surgeon and assistant worked together.
