Hello everyone,

I'm new to the coding and billing world. Our 3rd party billing company has stated in the body of the primary surgeon's operative report, a detailed description of what was performed by the assistant must be included in order for the assistant to bill for first assist. I have been unable to find this on CMS website. Does anyone have any references to this?
