Our Provider initially inserted the Mirena and then had to remove it because it was malpositioned. She then attempted to insert a Skyla because it was smaller and it did not work either and had to be removed. I am going to bill for both IUDs as failed attempts but I am not sure how to bill the 5800 and 58301. Can these be billed as 2 units since there were two insertions and 2 removals and do I use modifier 52 on the 58301 since there was not an initial removal?
