I have a surgeon who operated on a patient who had a previous surgery 6 days by different surgeon, not in our practice. That surgeon did a L3-4 laminectomy & discectomy for HNP. We did a L1-2 lami w/o facetectomy,foraminotomy or discectomy - new level - for stenosis and then we rexplored L3 and did further lami without facetectomy, foraminotomy or discectomy. I would assume to use 63005 for L1-2 but what about L3, 63005 says 1-2 levels.
