If the dr. performed a Removal of colon polyp by snare and another removal of polyp completely by biopsy (not hot) do I use 2 codes? thanks :(
 
Yes you would use the 45385 for the snare and 45380 for the biopsy, especially if the polyps are in different areas (if it was a complete removal are you sure your doc did not do an ablation?).
 
Just for reiteration, you will definately need to 59 the 45380. You also have to make sure that the polypectomies were carried out in completely separate portions of the colon. Per the CPT guidelines, you cannot report multiple polypectomies in the same anatomic site separately. Ie: polypectomy by snare, transverse colon, polypectomy with biopsy forceps, transverse colon. These would not be separately codeable. :0).
