I'm new at hospital coding.
If an MD puts in note that patient had sepsis, and writes next to it "resolved" do we bill sepsis? (as on the discharge summary)
If an MD lists Discharge Assessment, and follows it with Discharge Chronic Conditions, do we bill chronic conditions with assessment, or assessment only?
