I just need help coding this situation. Pt was seen in office for the following
99214-25
93351
93325
93320 It was decided based on results of the STET and symtoms he presented with that pt would have a LHC done.

Later that night pt was brought to the cath lab at the hospital and a 93458-26 was done, so my question is do I need other modifiers and should I drop all this to paper to submit w/reports? Thanks Nancy

No, having the -25 of the office visit should be all that's needed. I don't see any need for it to be dropped to paper and have reports submitted.
