Our vasc surgeon has been following a pt in the hosp for carotid stenosis w CVA. He gets a call stating the pt is going to need a trach placed. He has had mulitple discussions either over the phone or in person with the patient's family because the patient cant verbally communicate. They have decided to have the surgery then changed their mind at least twice now.
I know I cant bill two E/Ms for the same day. I was thinking either I could combine the documentation to get a higher level or should I try using one of the prolonged visit codes. I dont think I've had any success in the past getting the PV codes paid.
Any suggestions???
