Where can I find an accurate listing of HCCs? Right now I am only able to google a code at a time to see if the diagnosis has an HCC attached to it.

Hi Robin,

Here is the link to the CMS HCC tables. This links to the 2012 Payment Year tables. There is a sidebar that will have prior years tables, so be careful to pull the right Payment Year table as CMS updates it and adjusts for code changes from year to year.
