2014 text for ex of soft sub q tumors

Still confused with whether to use 114-- codes or 2---- codes for reporting ex of a mass the extends into the sub q tissue. New text states "resection of tumors CONFINED to sub q tissue". What if it is some sort of mass that extends into the dermis as well? Is it determined by deepest layer removed or solely from where it originated?
Any thoughts appreciated

I would need to know what the path report says before answering this one. The difference would be the pathology as well as the depth of the tumor. If the path comes back as a cyst, then you would not use the musculoskeletal codes but rather the 114 codes. Hope this helps
