will billing for these new/updated/revised cpt codes be date of service based or date of charge entry based? ex: pt comes to office for knee injection on 12/30/2014 but my billers don't get the charge ticket until 1/5/15 - will we use the injection code and the ultrasound code? or the new code with ultrasound included? thank you! melissa
