On the MDM Grid under the Amount and/or complexity of data to review and analyze for Category 3: Discussion of management or test interpretation with external physician/other qualified healthcare professional/appropriate source, would a specialist provider get credit if they document "the manage plan discussed/communicated with Dr. Make Believe (the pt's primary care provider)"? Keep in mind the provider is only sending the other provider a letter of the visit with the patient.
 
If the provider is only sending a letter to the other provider, that doesn't seem like "discussion" to me. If it were a consultation, I would count it as the return letter, but for an office/outpatient visit, I wouldn't consider that sufficient for discussion of management or test interpretation for data reviewed if I were coding or auditing the chart.
