Hi,
Please advise which of the following modifier is correct for 20550 done bilaterally:
1.) modifier Lt/ Rt
2.) modifier 50
3.) Bill 20550 x2 units
4.) 20550/ 59 on the second line

Dx. Plantar fasciitis bilateral

Procedure:
Aseptic prep, cortisone injection consisting of 1.5cc 1% lidocaine plain, 1cc dex 4mg and 0.5cc kenalog10 injected into area of maximum tenderness 'Plantar fasciitis' BILATERAL heel. Pt tolerated procedure well.

What were the J codes used for the injectate and number of units billed for the above bilateral injection?
1.5cc 1% lidocaine plain, 1cc dex 4mg and 0.5cc kenalog10

What J code should be used if doctor injected 1.5cc 0.5% marcaine and 1cc 40 mg/cc kenalog ?
J3301 - 2 units?
 
Lidocaine/Marcaine isn't billable;
J3301 for kenalog and J1100 for Dex. and units are usually 2 depending on how much is used.-
This is how we bill it and yes depending on payer 50 works as well as the XS modifier and RT&LT on most insurances.
