20552 & 20553 rt-lt-50 modifiers?

I have been billing the 20552 & 20553 without the RT,LT, or 50 modifiers as this is per the guidelines. But AR has stated that she has a few rejections for the anatomical modifier. has anything changed??? I am unable to locate and news that there is a change.
Thank you in advance for your help.
 
the denials are from Medicare. Which is very strange because the podiatry guidelines clearly state these 2 codes should not have the modifiers.
