What is the proper way to code 20610 when both shoulders and both knees are injected on the same day? The payer is Medicare. We are in Nebraska. We have tried 20610 50 (1 unit) on one line and 20610 59, 50 (1 unit) for the second line; this has been denied.

If they are particular about using the X mods, you might try using XS instead of 59. Also make sure that there is a different diagnosis pointer for each line. If all of that is correct, appeal with documentation.

The denial was duplicate. I thought the MUE was 3, but someone else said only 2.
 
what about when both hips are injected.... left and right in the bursa....would we bill 20610-50 or 20610-LT and 20610-RT? Or does it vary by payer?
