If a pathology report shows a gastric polyp and chronic gastritis, is it necessary to use both codes? It has been suggested to me that since the polyp is secondary to the gastritis, that it is not necessary to code both.
 
i was taught to code them both...just as a CYA precaution...claim will probably get paid with just one, but better safe than sorry
