when would you code 21930 and when would you code 1160x or 1140x instead?

The 1160x & 1140x codes are used for skin and subcutaneous tissue. 20000-29999 codes are used for the musculoskeletal system. Knowing how deep the surgeon went would help you to determine which system your coding would fall into. Look to your path report as well. That should also give you an idea of what you're dealing with.
