Does "The long head of the biceps tendon was located in the groove. It was cut and tenodesed to the upper border of the pec" support 59 modifier?

Physician wants to bill both codes. This is the only documentation of his biceps tenodesis in the op report. Would a 59 modifier be appropriate for billing?

If it's a Medicare patient, it cannot be billed no matter what.

If it's a private payor, you can use a -59 and the description is fine, but you'd generally want a separate diagnosis to help prevent denials.
