Wiki 24 AND 79 together??????

I have a surgeon who did a surgery on the left shoulder. The patient came back in for a post op visit with complaints with the right shoulder and the dr did an injection in the right shoulder. Can we bill:

99024
99213-24-25
20610-79-RT
J3301

????

I wasn't sure if the 24 covers the post op and the 25 covers the injection or if we need to add the 79 to the injection to show it was also separate from surgery.

Any advice would be greatly appreciated! Also any documentation to support would be helpful.

Drop 99024

I would drop the 99024 entirely. That will just cause confusion.

Yes, you need BOTH -24 and -25 modifiers on your E/M for the contralateral shoulder.

Yes, you need the -79 modifier on the procedure.

OH, and be prepared for a denial and to have to submit documentation.

We never acutally send out the 99024 but key it into the system for tracking purposes. I was thinking the coding was right but it just felt like SO many modifiers. :)
