Wiki 24 Unrelated E/M during post op

I am looking for more clarity on this modifier. I have been told that I take it to literally and even though is says unrelated it actually means "outside of what is typical post op care". I have never heard this before and can't find anything to support it.

I deal with Trauma surgeons. One day they may be dealing with a patient with a complicated lac that requires repair and then the next thing you know the patient developes a pneomothorax the following day. In a case like this I would append a modifier 24 (unless the patient needed surgery)
