Quick question for usage of 25/57 on OV.......

If a patient has an office visit, biopsy, shave, and the decision for major surgery is made and scheduled for the following day. Would you report the OV with a 25 and 57 modifier? 25 for multiple procedures and 57 for decision for major surgery (90 day global)? Is my thought process incorrect? Please advise:) Thank you!
