Our physician saw a patient today and ordered a nuclear stress test, 78452, in 6 months with office visit following the test. Since the office visit was post testing and testing planned, is it appropriate to add a -25 modifier to the office visit?
 
Modifier -25 would only be used if your Dr did a procedure with a xxx 000 010 global period on the same day as the E/M. The E/M also must be significant and separately identifiable from the E/M bundled into the procedure in order to justify Mod -25. Doesn't sound like a procedure was done so no modifier would be necessary
