-25 with tetanus immunizations

We have been having several denials for the E/M when billed with a tetanus immunization. Insurance carriers are requesting the -25 on the E/M. Is this something new? Is anyone else having this problem also?
 
I dont know where you are but I would check with your carrier and see the reason for the denial. I know MD and PA require a AT modifier in some cases. And I have also heard of the new CCI edit update bundling some immunizations in with Well visits. In which case you would have to append modifier 25.
