For a consult that ends up needing surgery do I add a 25 and 57 or just a 57, if its a 63090 cpt being billed after the visit.
 
I responded to your PM. Modifier 57 is correct if E&M is decision for surgery. 25 is not used on Major (90+ day) procedures. 25 is to be used for Significant and Separately identifiable E&M on same day as Minor (0-10 day) procedure./
