Has anyone ever seen a 26 modifier used with an E/M before? Looking at a Provider based outpatient clinic. I admit I am confused.
 
No you would never use the 26 on an E&M. In the outpatient setting the facility uses their E&M based on entirely different criteria, so they are both paid no need for a modifier to distinguish between the facility and the physician E&M
 
ok. I had never seen it before and I knew it had something to do with the fact that the facility side was billing out their own code too. I just wasn't sure if it really needed to be there to be paid.
