I have a patient that had 27502 to both legs, same surgery session.

Does get this billed as

27502-50
or

27502-RT
27502-59-LT

Also is 29505 (Application of splint) included?

I'd do the 27502-RT and 27502-59-LT myself. As a rule, we haven't been using mod -50 at my practice and it seems to be working for us.

I know that casting/splinting isn't coded for inital care in an office setting. We have a hand/wrist specalists who casts in office after Fx care. If it's inital, we don't include the application, only the cast itself. If he has to re-cast during a follow up, then we can include the application code.
