Need help on a surgery. Provider documented:
Arthroplasty of the second DIPJ, right
Given the recurrence of the cyst decision for arthroplasty of the second DIPJ. The extensor tendon was cut transversely at the joint and reflected. Then a sagittal saw was utilized to cut the head of the middle phalanx. Intraoperative imaging confirmed appropriate amount of resection. Joint was copiously irrigated and then extensor tendon and dorsal soft tissues repaired with Vicryl suture
There are multiple opinions on what it should be. Provider wants 28124 due to the excision of the head of the middle phalanx. Coder thinks it should be 28285 due to the excision of bone and cutting and repair of extensor tendon. 3M is giving 26535 but debridement is not being done, an excision was. I'm thinking it should be 28899 with either 28285 or 28124 as the comparable. Any help would be greatly appreciated!
